In one of the books by Gene Chapman there is a picture of a forged "antler slotting fixture" for, well, slottong antlers. I made this one last evening. Havn't used it yet but i think it wil do thr trick. Bart

Gene Chapman is one of my favorites. I do a lot of my knives in his 'Iron Age Primitive style.

Guessing the threads are cut with a die?

I'd guess the threads were either forged or cut with a torch (grinning) Ken

How exactly would one go about making threads in a forge?

Use a threading hammer. ^_^

I lost my threading hammer, I think I left it next to the elbow grease and headlight fluid!

Maybe misplaced by the inside threading hammer. Probably find both of them at the same place.
